How do I create a survey using Microsoft Forms?

Creating a survey using Microsoft Forms is deadly simple. If you have a Microsoft 365 account, you can start right away! Excel Surveys can be created through OneDrive. Click the Microsoft 365 App Launcher button and choose the OneDrive application.

Then, hit the New button at the top of the OneDrive screen and select Forms for Excel option.

When prompted, enter the name for the Form and hit the Create button. When directed to a new screen, add a suitable title under the Untitled field for your survey and add an image, if required.

Next, add a short description for your survey. Click Add to add a new question to the survey. You can choose to add Choice, Text, Rating, or Date questions.

As you can see in the image above, I’ve chosen the Rating system for my survey. So, enter your first question for the survey and select the symbol for your Rating system (Star preferably). Also, choose the Level from the drop-down menu.

Click the Add New button to add more questions for your survey.

Here, you can configure Settings or options for online survey responses like,

You can even change the Thank you message by replacing Your response was submitted with a different line of text. When done, click Preview icon in the upper-right corner of Forms screen.

Check both, the Desktop and mobile version of your survey questionnaire. If all looks good, hit the Ok button at the bottom of the Preview page.

Ask respondents to submit their responses. That’s all there is to it! Random read: How to share Word files on Facebook.
